Below is a sample list of insurance plans that we accept.  However, our relationship is with you, our patient, and not with your insurance plan.  Your insurance policy is a contract between you, your employer, and your insurance company. 

We strongly recommend that you contact your insurance company prior to your visit to confirm that your individual plan has the coverage needed for vision and the services you are seeking.

As a courtesy, we may provide you with an insurance estimate, but it is not a guarantee that your insurance will pay exactly as estimated.  Your insurance company and your plan benefits will determine the amount paid.  If your claim is denied or the estimated payment is not otherwise made to our company, then you are responsible for paying the full unpaid amount at that time.

Furthermore, your deductible and co-payment is due at the time of service.  The deductible and co-payment is the estimated amount not covered by your insurance plan.
